Make sure that all of your triggers are set to Active.

Also double check that your workflow in general is set to active as well.

Make sure to double check:

• Is trigger set correctly

• Is Filter Set correctly

• Is action set correctly

• Troubleshoot with notification in between (If you are not sure where the problem is, you can set notification between the steps, and see which one gets trough and which does not)

If you have large number of filters, and you want notification to be sent each time Visual workflow is triggered, you can simple create one filter with notification that will be executed after the trigger. You can do this simple by setting up filter that has matching value of @ or does not have matching value of @. Make sure to set condition to OR (When Any).
